REGISTER FOR A BIOMETRIC SCREENING
Register for an onsite Biometric Screening or download a Physician Results Form:
- Go to powerup.hess.com and click the Programs tab.
- On the left, filter your view to All.
- Scroll down to the Quest Biometric Screenings tile and click Start Now.
- Verify your information, then either Make an Appointment at the office location where you want to have your Biometric Screening or click Order Form to download the Physician Results Form to bring to your doctor.
Unable to attend an onsite Biometric Screening?
Download the Physician Results Form (use the instructions above) to take with you when you get lab work done at your doctor’s office. Completed forms must be uploaded to powerup.hess.com by July 31, 2020, to receive POWER UP points.
