What You Can Choose
Family Accident Insurance
You can purchase Family Accident Insurance for yourself and your dependents, which provides benefits if you or a covered family member dies or is seriously injured as the result of an accident.
FOR YOU
FOR YOUR CHILDREN
You can elect from $10,000 to the lesser of $1 million or 10 times your annual base pay in $10,000 increments
Optional Child Family Accident coverage is 15% of your coverage to a maximum of $50,000
FOR YOUR SPOUSE
Optional Spouse Family Accident coverage is 60% of your coverage
Optional Life Insurance
You can purchase additional Optional Life Insurance coverage for yourself, and you can also choose to cover your spouse, same or opposite sex domestic partner and dependent children. You can change your coverage anytime. Benefits are paid if the covered individual dies while covered.

FOR YOUR CHILDREN
You can elect 1 to 4 times your annual base pay up to a maximum of $1.5 million
The amount of coverage depends on the age of the child:
n Age 14 days to 6 months: n Age 6 months to 26 years: $100 per child $5,000 per child
FOR YOUR SPOUSE
You must elect Optional Life for yourself first. Optional Spouse Life coverage is 50% of your Optional Life coverage to a maximum of $50,000

One of the benefits of enrolling in Optional Life Insurance is that you can get free will preparation and estate planning through MetLife’s Hyatt Legal Plans. These services can cost upwards of $1,000 each. By taking advantage of this benefit, you can save a lot of money.
If you currently have Optional Life Insurance, you can call 1-800-821-6400 today to schedule your consultation. If you elect Optional Life Insurance for 2019, you can access this service after January 1, 2019.
